For anyone who has ever wished they could slim down one part of their body while gaining volume to another, there is living fat transfer. Ohio-based plastic surgeon Dr. David Hartman uses this sophisticated procedure to harvest living-fat cells from areas where they are plentiful—such as the abdomen or thighs—then introduce them to areas where an increase is desired such as breasts, face, and gluteal area (buttocks).

This body procedure requires significant skill since the fat cells must be treated gently throughout every stage of the procedure, from removal to refining to implantation, so the fat grafting remains viable and healthy. How Does Fat Transfer Work?

No matter a patient’s ultimate aesthetic goals, our basic steps of fat transfer are similar.

Dr. Hartman uses tumescent, high-definition liposuction for fat transfer procedures, which involves the introduction of a sterile anesthetizing solution to the tummy area, thigh areas, and/or love handles are—the areas from which the living fat is to be sculpted out. At this point, tiny, carefully placed (to be hidden by swimwear) incisions are made for the insertion of a very narrow, specialized tube-shaped wand called a cannula. The BeautiFill laser living-fat harvesting device is used to optimize the harvesting. Using a gentle back and forth motion and the fat-harvesting laser, fat is sculpted free with the wand and drawn into a special wash/filter receptacle. Great care must be taken to preserve the integrity of the harvested fat cells.

Once successfully harvested by tumescent, high-definition liposuction, the fat cells are purified—a process that removes other fluids so that only the cells remain. These cells are then strategically injected in multiple layers into areas where more lift and volume are desired, such as for beautification of the breasts or buttocks or face. Because the tissue is your own, allergies and tissue rejection are not a concern. Your body welcomes its own cells, which should soon become established in their new home. Since they are living, they provide a stable, natural-looking boost.

Fat Transfer for Breast Augmentation

While many women may associate breast augmentation with artificial implants, living fat transfer is a natural and effective alternate solution, especially for patients who want a modest increase—typically up to one cup size larger. It is also an ideal option for women who simply don’t want artificial implants or who have limited breast tissue to begin with, which could limit a surgeon’s ability to create a pocket to hold the implant.

Dr. Hartman offers only fat transfer—no implants—for augmenting breasts, drawing on his years of experience and artistic eye to introduce fat cells with precision to create a look of naturally fuller, shapelier breasts. Note that this procedure adds volume, form, and projection, but addresses the sagging of breasts to only a modest extent. More severe breast sagging is better corrected with a breast lift in addition to the fat transfer. Minimal-incision breast lift options can also be done with your fat transfer—this can be discussed at your consultation.

Fat Transfer for Buttock Augmentation

Known as a Brazilian butt lift, buttock augmentation with fat transfer involves adding volume to your backside, primarily to accomplish two things: to give your “rear view” a rounder, fuller, more athletic-looking projection, and to correct mild to moderate sagging. Buttocks can develop a deflated look over time, which is correctable with the introduction of viable fat cells.

Recovering After Fat Transfer

Both the donor and recipient areas will be somewhat sore in the days following the fat transfer, and some bruising and swelling will be apparent. However, patients are asked to resume light activity the very next day. Most of our clients return to their job/daily routines by the 2nd or 3rd day after the procedure. A full return to strenuous exercise and work should generally wait until 2 or 3 weeks have passed.

Because fat transfer involves several tiny incision sites—within 12 inches of where the fat is harvested, as well as near the areas where the fat is added back—Dr. Hartman works to disguise the tiny incision sites in natural creases, folds, and lines on the body where they will be hidden by swimwear to be only negligibly noticeable. Although the incisions are minimal and well hidden, we routinely treat them in the follow-up visit(s) with an incision-blending laser to further help make them be indistinguishable in most cases.

Every fat transfer package is unique, so cost is best determined after an initial consultation with Dr. Hartman. Factors that impact cost include the scope of fat reduction and locations targeted for fat transfer.
